This is a wooden toy that has some tiny spinning tops in the gourd, by SATO, Kouchi (1936-) who is a Yajiro style kokeshi craftsman.
The 1.5cm high spinning tops are mostly the same as a woman’s little finger nail!
Gourd is called ‘hyoutan’ in Japanese. ‘Spinning tops from gourd’ (hyoutan kara koma) means ‘unexpected luck’. It is a fortunate wooden toy with a high degree of craftsmanship.
The product on the gourd is a sake cup. This gourd resembles a sake bottle.

He was born in Shiroishi city, which is the Yajiro style kokeshi crafting area, in 1936. He started learning woodwork under the guidance of Heisaburo Numazu (1933-) who is a Yajiro style kokeshi craftsman. He became independent in 1977.
