A kokeshi has its own car. This wooden toy is called Fuku-kuruma 福車.
This fuku-kuruma was made by OKAZAKI, Yasuo who is a Naruko style kokeshi craftsman.
This kokeshi can move forward with its wheels. Fuku means ‘luck’ and kuruma is ‘car’ in Japanese.
This doll is possibly vintage. Due to age, this kokeshi has spots.

He was born as the first son of Jinji OKAZAKI (1929-2004) in Naruko town. He started learning woodwork and making kokeshi dolls under the guidance of his father from 1971.
